1- Attendance Policy and Punctuality:
All players are expected to arrive at the training facility at least 10 minutes before the start of their session. This ensures enough time to warm up and be prepared for the lesson.
a. Absences:
If a player is unable to attend a scheduled session, parents or guardians must notify SPHERE SOCCER as soon as possible.
Consistent attendance is crucial for development, and unexcused absences may affect the player’s progress.
b. Tardiness:
Players arriving more than 10 minutes late may be allowed to participate at the coach’s discretion. Repeated tardiness may result in a review of the player’s commitment to the program.
2- 24-Hour Notice Policy for Session Postponement & Cancellation:
a. Session Postponement:
If you need to postpone a scheduled lesson, please notify us at least 24 hours in advance. Postponements with sufficient notice will be rescheduled based on availability.
b. Session Cancellation:
To cancel a session, a minimum of 24- hour notice is required. Cancellations made with adequate notice will be eligible for a make-up session, depending on the academy’s availability and policy. If less than 24-hour notice is given, the session may be forfeited without a refund or make up option.
c. Emergency Situations:
We understand that emergencies happen. In such cases, please contact us as soon as possible. We will review these situations on a case-by-case basis and make accommodations if deemed necessary.
3- Communication Contact Information:
All notifications of absence, postponement, or cancellation must be made via Phone or Email Address.
a. Confirmation:
After submitting a postponement or cancellation request, a confirmation from SPHERE SOCCER is required to ensure the change has been recorded. If you do not receive confirmation, please follow up.
4- General Conduct Respectful Behavior:
Players are expected to show respect to coaches, teammates, and all academy staff. Disruptive behavior will not be tolerated and may result in removal from the session or program.
a. Dress Code:
Players must wear appropriate soccer attire, including cleats, and shin guards.
Waiver Agreement
I, the undersigned, as the participant or the parent/guardian of the participant, understand that participation in soccer lessons involves physical activity that could lead to injury. I acknowledge the risks involved and agree to assume full responsibility for any injuries, damages, or losses that may occur as a result of participation in the soccer lessons. I hereby release, waive, discharge, and hold harmless Nabil Chahine, and SPHERE SOCCER INSTITUTE, from any and all claims, liabilities, and demands that may arise from participation in the soccer lessons, including any injuries or accidents that may occur.
Medical Treatment Authorization:
In the event of an emergency, I authorize any representative of SPHERE SOCCER INSTITUTE to seek and obtain medical treatment for the participant. I understand that I am responsible for any medical expenses incurred as a result.
Code of Conduct:
I agree to adhere to all rules and regulations set forth by Nabil Chahine, and SPHERE SOCCER INSTITUTE, I understand that failure to do so may result in termination of participation without refund.
PHOTO / VIDEO RELEASE:
I consent to the use of any photographs or video footage taken during the soccer lessons for promotional or educational purposes by SPHERE SOCCER INSTITUTE.
Acknowledgment of Understanding:
I have read this waiver and fully understand its terms. I acknowledge that I am signing this agreement voluntarily and that it shall be binding upon me, my heirs, executors, and administrators.
